The organisation has used its normal operational processes and teams to run its projects, rather than taking a “project” approach.
Responsibility for managing the project deliverables, schedule, budget and risks is given to staff who have an existing workload and do not have specific project management knowledge to run the project effectively.
The project’s purpose is not communicated to people outside the project.
